Community Health Center, Inc. CHC in Connecticut has received a $1 million federal grant to help women transitioning out of the York Correctional Institution YCI in Niantic.

The funding, provided by the Health Resources and Services Administration HRSA, will support a new initiative called the Women in Transition Program.

This program is designed to assist women as they prepare to re-enter their communities after incarceration. YCI, the state’s only prison for women, will work with CHC to identify participants within 90 days of their release.

The program focuses on providing personalized care and support. It starts with assessing each woman’s needs and creating a detailed plan to ensure they receive healthcare and other vital services. CHC will help connect participants with primary care, mental health support, substance use treatment, and other specialized services.

After their release, CHC will continue assisting these women by linking them to local resources and removing obstacles to getting the care they need.

Kasey Harding, CHC’s director of the Center for Key Populations, highlighted the importance of helping women succeed during this critical phase.

This initiative will empower women to achieve their goals, strengthen their families and communities, and reduce the likelihood of reoffending, she said.

As Connecticut’s largest health center, CHC is confident in its ability to lead this program and provide the support needed for women to reintegrate successfully into society.
